Insurance Info

The majority of healthcare plans cover Occupation and Physical therapy services, and physical therapy doesn’t require a referral from a physician. We will need you to provide us with your most current insurance card and picture ID at your initial therapy appointment. As a courtesy, we will contact your insurance provider to try and obtain information regarding coverage for our services. Although we try to gather as much information as possible from your insurance provider, it is ultimately your responsibility to verify coverage for service. Together, we will do our best to navigate the ever changing landscape of health insurance.

Co-payments are due at time of service while deductibles, co-insurance and amounts that are not covered by your insurance will be billed to you and payable within 30 days of receipt. If you have a limited or high-deductible plan and would like to pay at the time of service, we are happy to provide an estimate of your costs.

Out-of-Network/Private Pay Rates

We offer a discounted private pay service rate of $195 for an initial OT/PT assessment, with follow-up treatments billed at $175/hour, payable at time of service. Clients with high deductibles or limited coverage for therapy usually find the cash rate less expensive than billing through insurance. We are happy to discuss this option with you further.

NOTE: No-shows and late cancellations, with less than 24 hours notice, will be charged the full Out-of-Network rates for the session.

Testimonials

I was told that my arm would never go over my head again. As an athlete, this was incredibly devastating news. I was told my long thoracic nerve was beyond repair. Aaron was able to spot dysfunctions in my movement patterns during my weight training, which was causing nerve traction and my long thoracic nerve to shut down. I’ve already had dramatic relief in pain and in my overhead movement ability!!!

Sam was great to work with and he efficiently got my shoulder back in shape after a dislocation by providing me with a specially-targeted exercise regimen. MoveMend does not use PT aides, which meant I had Sam’s complete attention for the full appointment time.

FAQs

Can I benefit from PT treatments?

Working with you and your doctor, we can discuss your options and help determine if physical therapy will be the right choice for you. Call us today to discuss your current condition or book an appointment now.

Do PT treatments hurt?

Physical therapy is not meant to hurt, but to relieve pain. However, as you restore your mobility, pain levels, at times can fluctuate. You might experience some temporary soreness from using muscles that are weakened or recovering.

This is a normal response to therapy treatment. Good communication with your therapist will help minimize any discomfort. We want you on your way to quick pain relief and back to normal activities.

Do I need to wear certain types of clothes for my appointments?

Dress comfortably with clothes that allow easy access for the therapist to examine your problem areas.

For treatments, loose fitting clothes that are comfortable for gentle exercises and supportive sneakers are often good choices to wear.

Do you treat my condition at your clinic?

Physical therapists have years of university-level training in anatomy, physiology, kinesiology (study of joint movement), neurology, and many of the other medical sciences.

Our physical therapists have graduated from accredited physical therapy programs and are state licensed. Depending on the university, the physical therapy program is either a masters or doctorate level degree.

Rest assured, your therapist is well trained to evaluate and treat your condition.
